Transplantation of the upper wisdom tooth to the place of the lower sixth tooth in the TOP1 Moscow clinic - German Implant Center
Patient is 30 years old . Haven't been to the dentist in a long time. He was diagnosed, made therapeutic sanitation. It was determined that the bottom six on the left (tooth 3.6) cannot be saved:
The six was severely destroyed due to caries, there was destruction of the bone tissue of the tooth with cracks in the crown and root parts. Before that, we had already removed one wisdom tooth with a follicular cyst in this patient.
In this clinical case, we performed autoimplantation of tooth - in place of the extracted tooth 3.6, we transplanted another, the patient's own tooth.
Today we will talk about the transplantation of a wisdom tooth in place of tooth 6. That is, in place of the extracted tooth, we will not put a classic titanium implant, but we will install a living tooth - our own eight 1.8 hidden in the patient's gum. Stereolithography will help us to insert a new tooth, the task of which is to help the wisdom tooth exactly fit into the place of the removed tooth, repeating all its configurations when connected to tissues.
We begin the description of this clinical case by showing a stereolithographic model of the tooth. This is the model of the upper wisdom tooth 1.8. Made on 3d printer from special plastic, the model completely repeats the root of the eighth tooth, which will be a donor when the figure eight is implanted in place of the sixth tooth.
Why did you choose the top eight implant?
The right upper eight more or less suited the patient in terms of the size of his lower left six. In general, in most cases of autoimplantation, we are talking about the transplantation of wisdom teeth in place of problem teeth of the chewing group - sevens, sixes, sometimes fives.
During the operation, we do not look at the fact that the donor tooth stands up perfectly - the main thing is that it fits in size. It is important that it does not have strongly crooked roots, because if the wisdom tooth is with crooked roots, it will not be possible to carefully remove it, the crooked wisdom tooth will have to be sawn. And we don’t need a sawn tooth, respectively. For example, such a wisdom tooth will not fit for autoimplantation:
If the edge of the tooth breaks off - it's okay, we will only complicate the life of the therapist a little, since it will be a little more difficult for him to fill the canal later, but in general, a small chip in the tooth is not critical. But it is better to keep the wisdom tooth intact.
We proceed to the atraumatic removal of the lower six
The problematic six is covered with a temporary filling: before we were going to remove it, a diagnostic preparation was carried out, after which it became clear that this six could not be saved. Having closed it with a temporary filling, they ensured for a while that food would not get into it. And they themselves began to prepare the patient for surgery.
By the way, it was because of the plaque on the teeth and the lack of regular professional hygiene that the patient began to exacerbate in the zone of tooth 3.6.

We start the implantation of a living tooth
Now it's time to install our model in the hole of the extracted tooth - a fit that repeats the shape of the root of the eighth tooth, which we talked about earlier. And the shape of this root does not yet correspond to the holes of the tooth, in which the roots of the six were located. Therefore, the holes at the site of tooth extraction are corrected with a burr until we obtain the required size and configuration for the stereolithographic model, which, in turn, ensures that the site for the new tooth is kept intact.
In the photo you can see that the tooth model is fitted with . The time from preparation to fitting takes from 5 to 10 minutes.
When we use a stereolithographic tooth, as soon as we have removed the problematic tooth, immediately after 10 seconds we begin preparing and installing the model. And only after that we proceed to the removal of the figure eight, followed by the implantation of a wisdom tooth in the place that the stereolithographic model now occupies.
Will the molar tooth take the place of the extracted one?
It is also important when implanting your own tooth that as soon as we extract the donor tooth with the root and with the periodontal ligament, we must immediately proceed with its installation in place of the implanted tooth. Since this place is currently occupied by a steriolithographic model, while my assistant extracts it, I had a few seconds to photograph the figure eight, our donor tooth :
Notice there is periodontal ligament around the tooth that we talked about earlier. Now the implantologist begins a hard time limit, no more than 20 minutes. Since if you do not promptly insert a new tooth in place of the removed one, then within 20 minutes this ligament completely dies. The periodontal ligament contains blood vessels, which then help the tooth to osseointegrate, grow in a new place. The tooth is fed from this ligament.
Therefore, each operation for transplanting an extracted tooth is preceded by a thorough preparation, which includes:
- computed tomography,
- preparation for the extraction of a donor tooth,
- making an exact model of the donor tooth on a 3D printer.
How atraumatically removed the top eight, donor tooth
The tooth we need is impacted top eight 1. 8 , while hidden, is under the gum. As you can see in the photo, so far a neat cut has been made at the location of the eight. This is how this tooth looks on a tomography:

Implantation of a living tooth in place of the lower six
You see that the artificial tooth is taken out, and in its place a live impacted figure-eight tooth is transplanted, which has been hidden in the gum all its life. Figuratively speaking, "the child was born into the world and immediately began his life in a new place." In any other case, this eight would have been removed someday and would not have been useful. But in this case, its role in the restoration of the patient's dentition is the main one, and due to the presence of a wisdom tooth in good condition, such a jewelry operation became possible.
The transplanted tooth is sutured and sealed in the gum so that there are no empty spaces around its root.
The installed tooth still does not reach the norm in terms of bite, but in the future this tooth is planned to be covered with a ceramic crown.
After 2 weeks , the NIC general practitioner removed the nerves in the implanted tooth under a microscope, and the canals were carefully sealed. In general, a nerve can take root in a transplanted figure-eight tooth only if the patient is less than 20 years old. This is the age when the roots of the wisdom tooth are not yet fully formed. And if the root has formed, then there is no longer a growth zone and, accordingly, it has no growth potential.
The result of implantation of a wisdom tooth in place of the six
The chance of taking root in such a tooth during autotransplantation is almost 100 percent . Why? Because the transplanted tooth has a large number of its living tissues, which fully correspond to the tissues of the patient's body. It is not a foreign body like a titanium implant. And absolutely its own part of the body, which, subject to the technology of transplantation, will not be rejected in the future.
The results of computed tomography show excellent survival of the transplanted tooth.
All 4 canals in the transplanted wisdom tooth were treated and hermetically sealed.
According to the results of observation after 6 months, it can be seen that the bone tissue is fully formed , there are no empty spaces in it. The bone around the tooth is identical to the bone around adjacent teeth.
